Good articles must be collected, wheelchairs, prosthetic aids how to buy comfort?

Various assistive devices have a compensatory function for the disabled, and these assistive devices will accompany the disabled throughout their lives. Due to the long-term use and dependence, these assistive devices must be compatible with the body parts of disabled people. Various types of assistive devices involve all aspects of life for people with disabilities. There are more than 3,000 commonly used assistive devices! To this end, the editor of the China Disabled Persons' Federation has listed the methods for purchasing assistive devices commonly used by people with physical disabilities, hearing impairments, and visual impairments. We selected wheelchairs and prostheses to explain to you disabled friends.

Purchase of prosthetic limbs

For people with physical disabilities, prostheses are a second life. They not only provide physical help, but also provide psychological comfort. Therefore, people with physical disabilities should be very careful when choosing prostheses and choose the one that suits them. But how can you choose a prosthetic limb that suits you?

1What are the principles for choosing prosthetics?

1) The principle of focusing on function:Assembling a prosthetic limb requires restoring the basic functions of the amputated limb. However, in some cases, such as when a decorative prosthetic hand is installed after partial hand amputation, but the feeling of the residual hand is lost and the residual function of the residual hand is hindered, it is not necessary to forcefully install it.

2) Pay attention to practical results and price-benefit ratio, and do not blindly pursue high prices: There are many types of prostheses produced domestically and imported from abroad, and the price gap is huge. When choosing, amputees should understand and compare the performance, characteristics and prices of related prostheses. Some prostheses are designed for certain special groups of people. For example, a thigh prosthesis called a smart leg produced in the UK costs tens of thousands of yuan. It is suitable for people who often need to walk quickly, but it is not suitable for the elderly. Some prostheses are specially designed for competitions and may appear unnatural in daily life.

3) Focus on the brand, including both the prosthetic limb assembly unit itself and the prosthetic limb components used. It is necessary to consider whether the installation unit has a complete pre-sales, sales and after-sales service system. The prosthetic components used should pay attention to factors such as quality, performance, and whether they have passed the test of time. Also consider whether it is easy to maintain and replace vulnerable parts. For example, some non-mainstream imported brands of prostheses do not have maintenance services in China. If they encounter serious damage, they must be sent abroad. The journey is long and the cost and time are very high, so it is not suitable to use them.

2. What details need to be considered when assembling a prosthetic limb?

1) Selection of prosthetic components:Choose prosthetic components that suit you based on your residual limb condition, physical and age conditions, economic status, living and working environment, etc. Ordinary amputees can just choose national prosthetic standard parts; amputees can choose prosthetic parts with special properties according to their own economic situation.

2) Socket selection:There are professional prosthetists in the prosthetic fitting center who will give some fitting advice. If the amputee's residual limb is normal, a resin or plate socket can generally be used. If the amputation is due to special reasons and the condition of the residual limb is not very good, such as the residual limb is too short, neuroma, etc., you can choose a silicone or gel socket.

3What are the requirements for children and the elderly to choose prosthetics?

When a child patient chooses a prosthetic limb, he or she does not necessarily have to choose a high-end prosthetic limb. Because children are in the stage of growth and development, depending on the development, the socket needs to be replaced once a year and the prosthetic limb needs to be replaced every two years, which is helpful for growth and development and can prevent deformation of other parts of the body during the growth and development stage. For example, habitual deformities of the pelvis and spine.

When elderly patients, especially those with amputations due to diabetes, vasculitis and other reasons, choose prostheses, they generally choose prosthetic parts with light materials and good joint stability or special prosthetic parts for the elderly. Because most elderly people wear prosthetic limbs to walk and do not do strenuous exercise (except for those with special requirements); patients with diabetes and vasculitis are recommended to choose special silicone or gel sheaths to protect the skin of the residual limb to prevent skin abrasion when wearing the prosthetic limb.

If the skin grafting and scar adhesion of the residual limb are serious, soft sheaths such as silicone and gel are generally used to protect the skin of the residual limb to prevent the inconvenience caused by skin abrasion during the use of the prosthesis. At the same time, it can reduce the burden of walking with prosthetic limbs.

4What are the indications, contraindications and complications of prosthetic limbs?

1) Indications: In principle, all amputees have indications for fitting prosthetic limbs. The current level of prosthetic limb manufacturing technology can enable any mutilated limb to be fitted with prosthetic limbs.

2) Contraindications: A. Poor peripheral circulation of blood vessels. It is estimated that use will worsen the blood circulation of the stump. B. The stump has inflammation, pain, neuroma, tight skin, etc. C. He is old and frail, and his mobility is very poor. It is estimated that wearing a prosthetic limb will not help his mobility.

3) Complications: After assembling a prosthesis, regularly check the structure and mechanical properties of the prosthesis for damage and pathological changes in the stump. The presence of unfavorable factors in the prosthesis itself (stress on the socket, incorrect alignment) often leads to complications such as pain, compression ulcers, neuralgia, bursitis, dermatitis, and osteophyte hyperplasia after using the prosthesis, which affects the wearing of the prosthesis and should be treated promptly. Long-term use of unreasonable load-bearing prostheses can cause skin abrasions and other complications, which should be resolved in time.

5What kind of training should be done before using prosthetic limbs for lower limb amputation?

Training before installing prostheses: Generally speaking, breathing exercises and limb-strengthening exercises should be performed in bed on the second day after surgery, and full-body adaptive training should be carried out on the 3rd to 5th day after surgery, including physical enhancement training, psychological preparation, and the production of prostheses.

1) Whole-body exercise training: Especially when the level of amputation is high, or when both lower limbs are amputated, and the patient is old, frail, sick, or in poor physical condition, it is even more important to strengthen physical training. You can choose various sports training programs suitable for patients, such as wheelchair basketball, sitting volleyball, pull-ups, upper limb resistance training, low back muscle training, etc.

2) Residual limb training: Residual limb sports training should be carried out as early as possible without affecting the effect of residual limb surgery. Calf amputees should perform isometric contraction training of the quadriceps as early as possible, thigh amputees should perform isometric contraction training of the gluteus maximus and adductors as early as possible, and forearm amputees should perform flexor and extensor muscle training and shoulder joint muscle training as soon as possible. After the hard bandage is removed, training to recover and increase muscle strength and joint mobility should be carried out as soon as possible under the guidance and supervision of a sports therapist. Start with freehand training and gradually increase to the use of sandbags, pulley traction and other resistance training. This is an important measure to prevent joint contracture and deformity, and also creates favorable conditions for wearing prosthetics as soon as possible.
