Prosthetics fitted through work-related injuries, how to apply for replacement of prosthetics after their service life expires

The editor learned from the social security department that according to the "Interim Measures for the Management of Assistive Devices for Workers Injured at Work", the assistive devices equipped for workers injured at work should be replaced regularly according to the service life specified in the "Standards". If it is necessary to replace an auxiliary device, the person or a relative must submit an application. If the Municipal Labor Ability Appraisal Committee confirms that the replacement conditions are met, the required expenses will be paid by the work-related injury insurance fund. If the required service life is not reached, the required fees will not be paid by the work-related injury insurance fund. For employees with level one to four work-related injuries who enjoy one-time work-related injury insurance benefits and those who have terminated work-related injury insurance relationships, the work-related injury insurance fund will not pay for the cost of repairing or replacing assistive devices.

When an employee who is injured at work applies for re-evaluation, he should fill in the "Application Form for Re-evaluation of Labor Ability of Work-Injured Employees" and provide the following materials:

(1) One copy of the work-related injury determination decision;

(2) A copy of the initial appraisal conclusion (if the initial appraisal conclusion is made after the 15-day application limitation period, proof of the time of delivery of the appraisal conclusion must be provided);

(3) A copy of the person’s ID card, a one-inch bareheaded photo and a photo of the injured part;

(4) The medical records, diagnosis certificates, inspection reports and other diagnosis and treatment materials or copies of the person being appraised;

(5) Other information required by regulations, policies or the Center.
